By joining this telehealth appointment with Ohio Functional Rehab, you confirm that you are physically located within the state of Ohio at the time of this visit. You acknowledge that telehealth services are provided in accordance with Ohio law and that your participation serves as your agreement to receive care through a secure, remote platform. If you are not in Ohio, please end this session and contact our office for guidance on appropriate next steps.
